This Privacy Policy explains how PredictiCare LLC, a Texas limited liability company ("PredictiCare," "we," "us," or "our"), collects, uses, and shares information about visitors to our public websites, including predicticare.health and any subdomains, landing pages, and content we make publicly available (collectively, the "Site").
1.Scope of This Policy
This Privacy Policy covers Site visitors only. It describes what happens when you browse our pages, request a demo, or email us.
It does not cover the Platform. Information handled inside the PredictiCare software platform (the "Platform") at app.predicticare.health, including any client, patient, caregiver, or agency records, is governed exclusively by the PredictiCare Platform Terms and Conditions and, where applicable, a Business Associate Agreement and order form. Those agreements, not this policy, define our obligations as a business associate under the Health Insurance Portability and Accountability Act ("HIPAA"). If you are a Platform customer, your agency's agreement with us controls.
Your use of the Site is also subject to our Website Terms of Use.
2.Do Not Submit Health Information
The Site is not designed or intended to receive protected health information ("PHI") or any individually identifiable health information. Do not submit any client, patient, or personal health information through Site forms, email links, or any other Site channel. Any such submission is made at your own risk, is not covered by any Business Associate Agreement, and we may delete it without notice. The Site is also not monitored for medical emergencies. If you are experiencing a medical emergency, call 911 or your local emergency number immediately.
3.Information You Give Us
We collect information you choose to provide. Today that is limited to the following.
- Demo requests. When you complete the demo request form, we collect your first name, last name, business email address, and optionally your company name, number of locations, and company website.
- Email. If you write to hello@predicticare.health, we receive your email address and whatever you include in your message.
- Anything else you send. If you send us feedback, ideas, or materials through the Site, we receive what you send. Feedback is treated as non-confidential, as described in Section 6.3 of the Website Terms of Use.
We ask only for business contact information. Please do not include personal information about anyone else, and in particular do not include health information about any individual.
4.Information Collected Automatically
When you visit the Site, our hosting infrastructure and any analytics tools we use may automatically record standard technical information, including your IP address, browser type and version, operating system, device type, referring page, the pages you view, and the dates and times of your visits. We use this to keep the Site secure and available, to understand which pages are useful, and to diagnose problems.
5.Cookies and Similar Technologies
The Site uses only what is necessary to function. We do not use advertising cookies, we do not run advertising pixels, and we do not build advertising profiles about you.
Where the Site sets or reads cookies or similar storage, it is for essential purposes such as remembering a preference you have set or protecting a form against abuse. If we later add analytics or other non-essential technologies, we will update this policy and, where required by law, ask for your consent before those technologies are used.
Most browsers let you refuse or delete cookies through their settings. Refusing essential cookies may prevent parts of the Site from working correctly.
6.How We Use Information
We use the information described above to:
- respond to your demo request, question, or message;
- schedule and conduct a product walkthrough with you;
- send you information about PredictiCare that you have asked for, and, where permitted, occasional updates about the product, which you can stop at any time;
- operate, secure, maintain, and improve the Site;
- detect and prevent fraud, abuse, scraping, and security incidents; and
- comply with law and enforce our agreements.
We do not use Site visitor information to make any clinical or care decision about any individual, and we do not combine Site visitor information with Platform records.
7.How We Share Information
We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We share information only in these circumstances:
- Service providers. With vendors who host our site, deliver our email, and provide similar operational services, under contracts that limit them to acting on our instructions. Section 8 lists the categories we use.
- Legal and safety. When we believe disclosure is required by law, subpoena, or other legal process, or is reasonably necessary to protect the rights, property, or safety of PredictiCare, our users, or the public.
- Business transfers. In connection with a merger, acquisition, financing, or sale of assets, in which case we will require the recipient to honor this policy or give you notice and a choice where the law requires it.
- With your direction. When you ask us to share information with someone else.
8.Service Providers We Use
The Site currently relies on the following categories of provider. We keep this list current as the Site changes.
- Website hosting and content delivery. To serve the pages and static assets you are reading now.
- Form processing. Demo request submissions are transmitted to our own systems for handling by our team.
- Email. To receive and reply to messages sent to our published address.
- [Analytics provider, if and when added] We will name the provider here and update Section 5 before any analytics technology is enabled.
9.Retention
We keep demo request and correspondence information for as long as needed to respond to you and to maintain a record of our business communications, and then for as long as reasonably necessary for legal, accounting, or dispute resolution purposes. Automatically collected technical logs are kept for a limited period for security and troubleshooting. When information is no longer needed, we delete it or de-identify it. You can ask us to delete your information at any time using the contact details in Section 16.
10.Security
We use administrative, technical, and physical safeguards designed to protect information against unauthorized access, disclosure, alteration, and destruction, including encryption of information in transit to and from the Site. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. This is another reason not to send health information or other sensitive personal information through the Site.
11.Your Choices
- Email preferences. You can opt out of product update emails at any time by using the unsubscribe link in any such message or by writing to us. We will still send you messages needed to respond to a request you made.
- Access, correction, and deletion. Write to us and we will help you access, correct, or delete the information you have given us through the Site, subject to any legal obligation to retain it.
- Browser controls. You can manage cookies through your browser settings, as described in Section 5.
- Global Privacy Control. Where the law requires us to honor an opt-out preference signal such as Global Privacy Control, we do so.
12.State Privacy Rights
Depending on where you live, you may have rights to confirm whether we process personal data about you, to obtain a copy of it, to correct it, to delete it, and to appeal a decision we make about your request. These rights are provided under laws including the Texas Data Privacy and Security Act, the California Consumer Privacy Act as amended, and comparable laws in other states.
To exercise a right, contact us using Section 16 and tell us what you would like us to do. We will verify your request using the information we already hold, respond within the time the applicable law allows, and will not discriminate against you for exercising a right. If we decline your request, you may appeal by replying to our response, and we will inform you in writing of the outcome and of how to contact your state attorney general if you remain dissatisfied.
An authorized agent may submit a request on your behalf where the law permits, with proof of authorization.
13.Children
The Site is intended for individuals who are at least 18 years of age. It is not directed to children, and we do not knowingly collect personal information from children through the Site. If you believe a child has provided us information through the Site, contact us and we will delete it.
14.Visitors Outside the United States
PredictiCare operates in the United States, and the Site is hosted and administered there. If you visit the Site from outside the United States, you understand that the information described in this policy will be transferred to and processed in the United States, where privacy laws may differ from those in your location.
15.Changes to This Policy
We may update this Privacy Policy from time to time. The "Last Updated" date above reflects the most recent version. If we make a material change, we will take reasonable steps to notify you, such as posting a notice on the Site. Your continued use of the Site after a change takes effect means you acknowledge the updated policy.
